You are eligible for enrollment if you are aged 18 and less than 69 and your account(s) is in good standing. Your Certificate, explaining all benefits and terms of coverage will be mailed to you upon enrollment and will indicate the effective date of coverage. You may cancel your coverage with no obligation within 30 days of receiving your Certificate. Disability Benefits and Terms: Benefits are paid as long as you are disabled and will continue until 24 consecutive payments have been made, or you return to work, or you are no longer disabled, or the outstanding balance has been paid, or the maximum of $10,000 has been paid (whichever comes first). If you have more than one account, the total insurance provided under all your accounts cannot exceed $10,000.
The monthly benefit will equal either 5% of your incurred outstanding balance or $10, whichever is greater, to a maximum of $500 per month. Coverage does not apply to purchases or charges made after you become diagnosed with a Critical Illness or disabled or while you are receiving benefits under this coverage.
For Critical Illness: Benefits are paid if you are initially diagnosed with one of the following illnesses or conditions: Heart Attack, Stroke, Cancer, Coronary Artery Disease (refers only to bypass surgery), while insured; and you survive more than 30 days after the initial diagnosis. Payment of a Critical Illness benefit terminates all coverages under this Certificate. The proceeds are the total amount incurred on Your Account as of the date of diagnosis, to a maximum of $10,000. Critical Illness does not cover a pre-existing condition, non-invasive cancers in situ, Kaprosi's Sarcoma, any skin cancer other than malignant melanomas, localized non-invasive tumors showing only early malignant changes, papillary cancer of the bladder, and stage 1 Hodgkin's Disease; or stage A prostate cancer; or transient ischema attacks of vertebrobrasilar ischemia.
Disability does not cover losses caused by normal pregnancy or childbirth; intentional self-inflicted injuries, attempted suicide, attempted commission of a criminal offence; nervous, mental, or psychological, emotional or behavioral disorder; abuse of drugs or alcohol; foreign travel or residence; or a condition that required medical diagnosis or treatment in the six months before your coverage began, if the loss occurs within the first six months of coverage.
Life Insurance Terms and Benefits:
Payment will be made in the amount of your total outstanding balance at the time of death to a maximum of $10,000. If you have more than one account, the total insurance provided under all accounts cannot exceed $10,000. Suicide during the first six months of coverage is excluded.
AMEX Account Protector Insurance coverages for Life, Critical Illness and Disability.
In order to file a claim, notice of loss must be provided to the insurer. Any claim must be filed to the insurance company after the 30 day elimination period and within 90 days from the date of loss. The insurer will send you a claim form for filing within 15 days after notice of claim has been received. Termination of Policy:
The insurance policy terminates on the sooner of:
a) The date of your request (in writing or by phone); or
b) The date your credit privileges have been revoked on your account; or
c) The date of the statement following your 70th birthday for Life, Critical Illness and Disability coverages.
d) The date of your or your spouse's Dismemberment or Death; or
e) The date your account is terminated
Coverage costs only 79¢ per $100 of your ending monthly Credit Card statement balance.

All the provisions of the Plan are contained in the Master Policy No 010206L. The Master Policy is complete in detail and the final interpretation of any specific provision or claim is governed by it. The terms and conditions of the Master Policy will prevail.
