An American Express Cardmember ("Cardmember") for the purpose of this Offer means a person holding a Card issued by American Express Banking Corp. in India.
• The offer would be valid on above mentioned cards issued in India only.
• This program is being offered by the participating service establishment only and shall be valid for the period mentioned in the offer.
• The offer in this program will be fulfilled at the merchant’s end only.
• This offer is being made purely on a “best effort” basis. The Cardmembers are not bound in any manner to participate in this offer and any such participation is purely voluntary.
• American Express is neither responsible for availability of services nor guarantees the quality of the goods/services and is not liable for any defect or deficiency of goods or services so obtained/availed of by the Cardmembers under this offer. Any disputes with regards to the quality of goods/services availed shall be taken up with the merchant/service establishment directly. American Express shall have no liability whatsoever regarding the same.
• American Express shall not be liable whatsoever for any loss/damage/claims that may arise out of use or non-use of any goods or services availed by Cardmember under this offer. American Express reserves its absolute right to withdraw and/or alter any of the terms and conditions of the offer at any time without prior notice.

• Priority Appointment Slots: Appointment will receive top priority, which means that Cardmembers will be able to schedule their appointment at a time that is most convenient for them. (Subject to availability of doctors)
• Personalized Care: Cardmembers will be assigned a dedicated healthcare attendant who will be responsible for their well-being during their visit to hospital. 
• Wellness Lounge Access: As a VIP customer, Cardmembers will have access to Lilavati lounge, where they can wait while waiting for their next appointment.
• Post-Checkup Follow up: Lilavati’s dedicated team will also follow up with Cardmembers post-check-up to ensure that the treatment prescribed is being followed and if any medical assistance is required in future.

The Card member will receive the email regarding the dedicated relationship manager once the appointment is fixed.

• The specialized rates are applicable only on payments made at the hospital payment counter using an eligible American Express® Cards at LILAVATI HOSPITAL & RESEARCH CENTRE - A-791, A-791, Bandra Reclamation Rd, General Arunkumar Vaidya Nagar, Bandra West, Mumbai, Maharashtra 400050.
• The discounts on OPD investigations, consultations, medicines, and health check-up packages can be available during the same visit.
• The discounts are applicable only on the hospital's standard tariff only.
• The room upgrade benefit is applicable only on fresh admissions.
• The complimentary Home Sample Collection is available only within 30 kms serviceable area
• The Priority Appointment, Dedicated Relationship Manager, Wellness Lounge Access, and Post Check-up Follow-up are available only for the Platinum Cardmembers and their immediate family.
• The discount on medicines is applicable only on medicines purchased from the hospital pharmacy. Discounts will be only applicable on the final invoice.
• Prebooking for Health Packages is compulsory. 
• There is no minimum transaction amount required to avail the offer.
• There is no stated maximum capping on the discount amount.
• The offer is not applicable on EMI transactions.
• The offer can be availed multiple times during the offer period.
• The offer is also applicable on Supplementary Cards.
• Cardholders/dependents must furnish copy of valid government identification card along with relation proofs for the dependents for records and always carry the same while availing the Services. 
• Lilavati shall have the right to verify the authenticity of the documents presented by the Beneficiaries. Lilavati shall reserve the right to refuse the Discount to any Cardholders/Dependents in case requisite documents are not furnished or their authenticity and/or validity is questionable.
• The discount would be applicable on the total bill (exclusive of taxes).
• All disputes relating to billings, offer, services etc. shall be resolved at the Lilavati hospital only.
• Cardmembers and/or their immediate family members are required to keep and present the receipt in case of any non-fulfilment related concerns.
• These specialized rates cannot be clubbed with any other offer or discount running at the hospital.
• Blackout dates are: National Holidays and Sundays 2nd October 2026 and 26th January 2027)
• Queries related to the offer will be entertained only till 15 days from the transaction date. Cardmembers will have to write to Ms. Renu Kakad at renuk@lilavatihospital.com / finance@lilavatihospital.com. Please expect a response within 24-48 hours of the time the query is raised.
• The specialized rates are applicable for American Express Cardmembers as per the offer description & their immediate family members. Immediate family members consist of spouses, children, and parents and valid relation proof required at the time of transaction.
• The offer would be extended subject to the availability of the doctor/service/ room at the hospital as per the hospital’s policy.
• The Offer Tariff would be applicable only on the services covered in the offer as per the hospital’s policy (Subject to Revision).
• The offer is non- transferrable, non-cashable and non- negotiable
• LILAVATI HOSPITAL and American Express reserves absolute right to discontinue/withdraw and/or alter any of the terms and conditions of the offer at any time without prior notice. 
• Lilavati Hospital is solely responsible for consultation, diagnosis, treatment, and patient care.
