Avail Executive Health Package worth INR 35,000 at a specialized rate of INR 12,250.

Offer Validity:  The Offer is valid from 29th August 2025 to 12th August 2026 (Both days included)

Offer Details:

• The Offer is also applicable on Supplementary Cards.
• Specialized rates are inclusive of taxes, Cardmembers do not need to pay taxes separately. 
• This Offer can be availed multiple times during the offer period.
• The specialized rates are applicable only on payment made at payment counter of Jaslok hospital and not online payments made on Jaslok Hospital’s App/ Website.
• Prebooking at Health Check Up Unit for Executive Health Package is compulsory 48hours in advance.
• This Offer would be extended subject to the availability of the doctor/service at the hospital as per the hospital's policy.
• Offer is not valid on home collection service.
• There are no specific exclusions in the executive Health Package. However, any additional consultations or investigations advised post-package evaluation will be billed separately as per standard hospital tariffs.
• Cardmembers must call out which American Express Network Card they hold.
• The Health Check Up Unit is open from Monday till Saturday except Sundays/Public holidays.
• Specialized rates would be applicable only for eligible Cardmembers & their immediate family members. Immediate family members consist of spouse, children, and parents. (Proof of relationship can be shown in the form of Birth Certificate and Aadhar card).
• The specialized rates cannot be clubbed with any other offer or discount running at the hospital.
• All disputes relating to services, billing, or offer fulfilment must be addressed directly with Jaslok Hospital.
• Cardmembers and/or their immediate family members are required to keep and present the payment receipt in case of any non-fulfilment related concerns.
• There are no exclusions in the exclusive Health Package.
• Queries related to the offer will be entertained only till 15 days from the transaction date. Cardmembers will have to write to the dedicated SPOC (as mentioned below) of respective hospital via email. Please expect a response within 24-48 hours from the time the query is raised.
• Standard Jaslok Tariff would be applicable on all other services as per hospital's policy (Subject to Revision). 
• Last date to raise any queries concerning this offer is 27 August 2026.
