If you're applying for a Personal Card, you'll be asked to provide the following information:
- Name
- Date of birth
- Home street address
- Your Social Security Number
- A valid email address
- A valid phone number
- Your annual income and source of income
- The full name and Social Security Number of anyone you wish to add to your account or give an Additional Card to.
