Download these forms to perform tasks not currently available through American Express @ Work, and fax to the American Express PA Servicing Team. Fax number: 905-474-8982, or 905-474-7958.

Due to Federal Government Regulations Paper-based applications have been removed from the Resource Center.

To submit Cardmember applications you can:

  • Provide your Cardmember with an access key to apply for a Card online (Instructions can be found by searching for "Access Key" in the Getting Started Guide)

  • Refer to the mailing sent to you the week of June 16th-20th that provides you with the link* to the new paper based Cardmember applications.

    *If you cannot locate this mailing simply contact the PA Servicing help desk at 1-866-568-0308 to have them email you a new form.
