How do I change my name on my Card and my Account?

If your name recently changed, you can request to have the details updated. Please complete this online form and upload to our online Document Submission Centre.  

 

Alternatively, you can download the form and send it to us via post. Once we receive your request and your name change has been updated on your Account we can issue a new Card that reflects your new name.
