• Fax us the certified true copy of your NRIC/passport or deed poll for name change for verification, with signed letter to 62954300 (or +65-62954300 if you are faxing from overseas).
• Mail us the certified true copy of your NRIC/passport or deed poll for name change for verification, with signed letter to Card Member Services, P.O. Box 852, Singapore 911912.
